TITLE(s):   Low-Density Lipoprotein Cholesterol Bulk Is the Pivotal
             Determinant of Atherosclerosis in Familial
             Hypercholesterolemia.

  Summary:    The low-density lipoprotein (LDL) cholesterol level and
             duration of exposure to the elevated LDL cholesterol level
             or "LDL bulk" are the privotal determinants of
             atherosclerosis and are the strongest predictors of carotid
             intima-media thickness in subjects with familial
             hypercholesterolemia. Other biochemical risk factors such
             as fibrinogen, lipoprotein(a), homocysteine, insulin, and
             plasma 8-isoprotstane, as well as LDL particle size are of
             little added predictive value.
